Patient Safety Movement: Zero Preventable Deaths
About: Founded in 2012, the Patient Safety Movement Foundation (PSMF) is the only non-profit 501(c)(3) that has publicly committed to eliminating preventable patient deaths in hospitals by the year 2020 (0X2020™). At the same time, they are setting up Regional Networks to duplicate efforts abroad. Medical errors are one of the leading causes of harm and death across the globe. The Foundation has worked with the world’s leading clinicians, hospital CEOs, healthcare technology CEOs, patient advocates and government leaders to create 30 Actionable Patient Safety Solutions (APSS) under 16 umbrella topics. These APSS are made available to every hospital and clinician free of charge. For patients, a free mobile App, PatientAider, empowers their loved ones to better navigate their hospitalization. The Foundation also convenes the annual World Patient Safety, Science & Technology Summit. Speakers and panelists include clinicians, researchers, patients, world leaders, senators, ministers of health and innovators from around the world.

The Patient Safety Movement Foundation has developed 30+ solutions to over 17 overarching patient safety challenges facing hospitals today. Every year these evidence-based solutions are updated by multidisciplinary workgroups to ensure the latest science, literature, and technology are provided.
About the association
Since 1976, the Louisiana Association for Healthcare Quality has been a guidepost for healthcare quality professionals. We were founded in 1976, later incorporated in 1982, and officially named the Louisiana Association for Healthcare Quality in 1995. LAHQ enjoys a diverse membership that includes professionals from many quality-related disciplines such as utilization, risk, case management, patient safety and infection control.
